Funding is available for Chester County Health Care Partnership organizations to help offset costs to attend the Pennsylvania Best Health Care Practices Conference Redefining Excellence, presented by the PA Center for Health Careers to take place in Harrisburg on April 15, 2010. Grant assistance would cover the Pre-Conference Tracks ($20) and Conference Registration ($75). Organizations will be required to document at least a 25% cash match, and will be responsible for covering all meals, travel or lodging.

Applicants must be willing to meet the grant requirements which include providing their social security number, registering their organization with Pennsylvania CareerLink and submitting a post-conference evaluation form. 

Chester County Health Care Partnership partners who have previously attended the Best Health Care Practices Conference consider it a highlight of their year. Participants will have the opportunity to network with education and industry experts in their respective disciplines and to share best practices and learn best practices conducted within the Commonwealth.

1)     Complete in full the Chester County Health Care Partnership Grant Application on page two of this document.

2)     Fax or mail the Chester County Health Care Partnership application to:  Fax: 610-458-7770 Attn: Patti VanCleave or via mail: Patti VanCleave; c/o Chester County Economic Development Council; 737 Constitution Drive; Exton, PA 19341.  (Please do not send electronically due to the confidential information provided on the application form).

3)     Applications will be accepted on a first-come, first-served basis until all funding is allotted. The application form is available for download via the link below.

Applicants will be notified of the acceptance of their applications within 2 days of receipt.

4)     Approved applicants must register and pay for the conference.  Registration for Pennsylvania’s Best Health Care Practices Redefining Excellence conference is available only at www.Ex-Dir.com.

5)      Grant awardees are required to provide their original Best Health Care Practices Conference registration with documentation of payment prior to their request for reimbursement.  Please allow 6-8 weeks for reimbursement.

6)     After the conference, grant awardees must provide a special post-conference evaluation form to Patti VanCleave or Cyd McCord.  Funding will not be reimbursed until all requirements of the financing agreement are met.
